Appointments & Scheduling
Scheduling Your Visit

Appointments may be requested online or by contacting our office directly. A member of our team will confirm your visit, review any required documentation, and provide preparation instructions when applicable. Appointment confirmation details and arrival guidelines will be clearly communicated prior to your visit to ensure an efficient check-in process.
Arrival Time
New patients, please arrive 30 minutes prior to your scheduled appointment time to allow time for check-in and any required documentation.
Established patients, please arrive 15 minutes prior to your scheduled appointment.
Late Arrivals
Patients arriving 15 minutes late may need to reschedule to ensure appropriate time for evaluation and care.
Cancellations & Missed Appointments
Cancellation Policy
We request at least 24 hours notice for cancellations or schedule changes whenever possible.
Missed Appointments
Missed appointments without prior notice may result in a missed-visit fee. This policy allows us to remain available to other patients in need of care.
Insurance & Payment Policy
Insurance Verification
Insurance coverage depends on your individual plan and the services provided. Our team works to verify coverage details prior to your visit when possible.
Financial Responsibility
Patients are responsible for co-pays, deductibles, and non-covered services at the time of visit unless prior arrangements have been made.
Surgery patients will be required to pay the professional fee for surgical services at their Pre-Op appointment.
Accepted Payment Methods
We accept major credit cards and other standard forms of payment. Please contact our office with specific billing questions.
Personal Injury & Documentation
For personal injury cases, clear and objective medical documentation is provided when clinically appropriate. All evaluations, findings, and treatment plans are based on medical necessity and professional judgment.
The practice does not provide legal representation or guarantee case outcomes. Patients are responsible for providing accurate accident details, insurance or claim information, and attorney contact information at the time of scheduling.
Prescription & Medication Requests
Prescription refills and medication requests should be submitted during regular business hours. Certain medications may require follow-up appointments prior to renewal. We do not prescribe medications without appropriate clinical evaluation.
Communication & Patient Portal
Secure Communication
Routine communication may occur through our patient portal when available. Sensitive medical matters should not be submitted through unsecured email.
Medical Emergencies
If you are experiencing a medical emergency, call 911 or seek immediate emergency care.
Patient Privacy Policy & HIPAA Compliance
Patient privacy is a core priority. We comply fully with all applicable healthcare privacy regulations, including HIPAA standards as outlined by the U.S. Department of Health & Human Services. Protected health information is accessed only by authorized personnel and used solely for treatment, payment, and healthcare operations as permitted by law.
Clinical discussions, records, laboratory results, and communications are handled with strict confidentiality safeguards. We do not share personal health information without legal authorization or your written consent.
